Residency, Enrollment and Health Forms
The following forms are available for viewing with Adobe Acrobat Reader. These forms must be completed by anyone registering in Ridgewood Public Schools. The forms can be downloaded and printed for your convenience. Please bring completed forms to your neighborhood school or to the school your child will be attending.
